About Skilled Nursing Facilities
Skilled Nursing Facilities specialize in skilled care that can only be safely and effectively performed by, or under the supervision of licensed healthcare professionals. Summa Health has secured care agreements with this type of facility to allow our care team to provide care at these facilities, promoting seamless transitions of care and overall better outcomes for patients.Patients in this level of care require daily skilled care by a skilled nurse and/or therapist due to a hospital-related medical condition. The types of conditions that require skilled care may include a planned surgery or procedure like knee or hip replacements or an unplanned, acute medical issue like infection, sepsis or cellulitis.
Skilled Nursing Facility Services
Skilled Nursing Facilities offer 24-hour care and therapeutic rehab as needed to treat, manage, observe and evaluate a hospital-related condition. Depending on each patient’s specific needs, the individualized treatment plan may include interventions provided by any or all of the following:
- Physicians and Advance Practice Providers (APPs)
- Skilled nurses
- Physical therapists
- Occupational therapists
- Speech-language pathologists
- Medical social services
- Medication management
